Start the level and swim through the water to reach Secret #1, continue through the water to reach the Gold Key in the sewer-like area. After you get the key you're done with the sewer area, go up the elevator and continue through the citadel until you reach the room with Secret #2. Inside this room you'll also find the Gold Key Door, it's not required for progression though. You'll find this door in a small cubby in the wall.
Keep following the primary path through the level until you reach a fork; one path dead ends with a Biosuit. The other path leads to a button and some acid water, hitting the button gives you a ledge that you can ride over the acid water but you'll want to make use of the Biosuit and swim.
Before you leave I recommend grabbing the Pentagram of Protection from Secret #3. Swim through the water and you will pass an opening on your left with a bunch of enemies inside about 3/4 down the river. Immediately after this opening in the corner of the acid water you can find another Quad Damage. It's recommended you grab this Quad Damage and go HAM on the enemies in the next room.
After clearing the next room follow the path to a button, hit this to bring down the gates near the Silver Key and grab it. Grab Secret #4 which is inside this room too, you'll find a Biosuit here which will allow us to cross the remaining acid water undamaged. This time you'll want to go down to the very end of the river, keep blasting enemies and open the Silver Key Door.
On the other side of this door you'll find a large room with enemies, hit the two buttons and ride the elevator down. In the next room you'll find the exit portal; to use it though you'll need to shoot the three buttons on the opposite wall. Look up if you're having trouble finding them!
Secret #1
Secret #1: When you reach the large open room where you must swim through the water you'll see a zombie standing on a platform at the far end with a Nailgun. Right next to this platform will be a hole in the floor with some Ammo/Armor and Quad Damage.
Secret #2
Secret #2: Above the first secret you'll see a bridge that goes over the entrance to the castle. You will go across this bridge later in the level, when you do go down the stairs in the next room and shoot the wall in the corner of the stairwell. This will reveal another secret location.
Secret #3
Secret #3: In the same room as Secret #3 you'll find a wall that you can shoot which reveals this secret area. You'll need to jump across a small gap to reach this secret. This secret is found on the wall opposite of the Gold Key Door, it's above the door and can be reached from the platform that the door goes under. Inside this secret area you'll find the Pentagram of Protection.
Secret #4
Secret #4: Jump into the acid water in the room with the Silver Key, you'll find a small alcove down here which contains a suit of red armor and another Biosuit.
