When the level begins follow the path to the room filled with water. It's recommended you explore a bit and clear all enemies then swim to the bottom and surface in the underwater building. Grab the Super Nailgun and jump across the water gap to reach a new area. Follow the linear path to a door with a pentagram design on it, this door leads to the Silver Key.

After picking up the Silver Key go straight into the next room and use the key on the door. You'll eventually go up an elevator to a room with 5 Quake symbols on the ground; you need to step on all 5 to activate the switch. Trap doors will open on either side after you step on the final switch, on harder difficulties enemies come out.

If you're interested in the second exit for this level, fall down into the holes that enemies come out of and press the switches. Read more below at Secret #3; otherwise continue up the stairs and to the exit up top.

Secret Exit Portal

 

Secret #1

Secret #1: At the start of the mission you are facing a small circular room; our first secret is in here. Shoot both switches on opposite sides of the wall to lower a platform with a yellow suit of armor on it.

 

Secret #2

Secret #2: Right after the room with the previous secret you will find a room filled with water. After entering this room go right immediately and search the wall below the water's surface, you'll find a small cubby that you can surface in - this is the secret area.

 

Secret #3

Secret #3 (Secondary Exit): Later in this dungeon after passing through the Silver Key door you'll find a puzzle room where you must step on 5 different Quake symbols all while being shot at by wall traps. After you solve this puzzle use the rooms that open up to drop down into the room below you. You'll find two switches on each platform, press both and you'll open this secret area. This secret area is located in the same room as #2, on the opposite side of the room you'll find a large opening you can swim into then surface in.
