Video: How to get Spear of Bitter Mercy
The Spear of Bitter Mercy is your reward for completing Sheogorath's Quest, a quest given to you by the Daedric Shrine of Sheogorath in the St. Delyn Canton, Ihinipalit of Vivec. Once you are in Vivec make your way to the St. Delyn Canton Waistworks and from here you can get to the St. Delyn Canalworks which will take you to the Underworks. Once you're in the St. Delyn Canton Underworks you'll be able to visit the Ihinipalit Shrine in the northern portion of this area. Interact with the Statue of Sheogorath in this shrine (pictured above) to begin the quest. What you need to do for this quest is to travel to Big Head's Shack and talk to him about the Fork of Horripilation. Big Head's Shack is found on an island to the northwest of Dagon Fel, the northern most town in Morrowind.
Take the Fork of Horripilation off his table after you've spoken to him and head to the island east of his shack with Nelas' Ancestral Tomb on it. Here you'll find a couple Bull Netchs but the one we're after is named Giant Bull Netch. Equip the Fork of Horripilation and kill the Giant Bull Netch with it.
You can wittle down his health first with a real weapon but if you do this I strongly recommend you Save first because if you accidentally kill him you can't complete the quest.
Once you've defeated the Giant Bull Netch return to the Statue of Sheogorath in Vivec to turn it in. He'll reward you with the Spear of Bitter Mercy, one of the best spears in the entire game.
