Skull Crusher is a powerful 1h blunt weapon which can be found inside of the Forgotten Vaults of Anudnabia; a dungeon which is accessed through the Omaren Ancestral Tomb. The entrance to the Omaren Ancestral Tomb is found on a small island east of Sadrith Mora, use my map screenshot below if you need further guidance.

The Omaren Ancestral Tomb is very small and you'll quickly find a passage that leads you into a massive room with water and tight walkways. Levitate is recommended here but with careful navigation and high enough Acrobatics/Jump you'll be able to make your way to the door that leads you to the Forgotten Vaults of Anudnabia, Forge of Hilbongard.

In the Forge of Hilbongard zone you'll need to open a lock level of 85 to get into the large room with lava. Then you'll need to use Levitate and open the locked chest floating in midair above the lava (pictured above) which has the Skull Crusher in it.
