To acquire this helm you'll need to complete Malacath's Quest. You can start this quest by speaking with Malacath's Statue inside Assurdirapal which is a dungeon found along the chain of islands on the northern end of the world map. Use my screenshot below if you'd like to see the exact location.

Inside Assurdirapal all you need to do is interact with Malacath's Statue (pictured above) and you'll get the journal entry for this quest. Our next destination is the town of Gnaar Mok which is found far to the southwest of where the quest starts.

Once you make it to Gnaar Mok you'll find our target, Farvyn Oreyn to the south. He's found just outside of the Shurinbaal dungeon and has a couple of Bull Netch corpses surrounding him. Kill him then return to Malacath's Statue inside Assurdirapal to complete the quest and receive the Helm of Oreyn Bearclaw.
